A care-plan builder for nursing students and working nurses, built around traceability rather than output. You bring the case material and it produces an evidence chain: cues, priorities, goals, interventions, rationales, sources and evaluation, each step connected to the one before it so you can explain why any element is there.

The workflow starts with a confirmation pass where you correct uncertain details before they shape the plan, which matters because errors early in a care plan propagate through everything downstream. From there you build and edit the chain directly, then review for gaps, revisit the evidence behind each proposal and export the revision you intend to use.

Two contexts are handled separately. For coursework, you bring the assignment brief, rubric and citation style so the plan matches what is being marked. For practice, you use a local template and policy, update only what changed, and keep a revision history against the existing patient context.

The design constraints are the notable part. It keeps uncertain details visible rather than smoothing them over, never invents missing patient facts, and links each proposal back to the case and the selected evidence. A free chain-check tool is available without signing in or entering any patient information.

Two things to know before you start. The iPhone app is still in App Review, so the web workspace is the product today. And the company states plainly that this supports care-plan organisation and review rather than replacing professional judgment, so generated content needs verifying against the current case, local policy and your approved references.
